OSHA Form 300A is summary of work-related injuries and illnesses required at all companies with 11 or more employees. Use these refills for your OSHA 300 Information Center kit (item #01355) or binder (item #01358).  Forms 300 (item #01359) and 301 (item #01361) are also available. All are sold 25 per package and measure 8.5" h x 14" w. They are not prepunched for binder use.

These forms need to be hole punched and folded in half to fit into a standard binder. As they are printed in landscape format, they will only span two rings of a standard 3-ring binder.
